List form submission rows
GET/form-submissions
A submitted form: answers against one template version, score, GPS stamp, signature and approval. Returns a cursor page of form submission rows the caller may see. The default order is `created_at` ascending. Supports sparse fieldsets and the filter grammar.
Create a form submission

A submitted form: answers against one template version, score, GPS stamp, signature and approval. Creates a form submission. Requires `Idempotency-Key`; a replay returns the original response. The organization is derived from the credential or the parent row, never from the body.
Get a form submission
GET/form-submissions/{id}
A submitted form: answers against one template version, score, GPS stamp, signature and approval. Returns one form submission with its `ETag`. A form submission the caller may not see returns 404.
Approve or reject a submission
POST/form-submissions/{id}/decide
A holder of the form's approver role decides a submitted form; never the submitter. A rejection needs a note. Requires `Idempotency-Key`; a replay returns the original response. A rule that refuses the request returns 422 with `refusal` and `reason`.
